About the role

This role sits in the Connected Communities team as part of the Community Life division which strives to develop healthy, active, connected and safe communities through the delivery of community facilities, events, experiences and life cycle services from birth to older age.

In this role you will:

  • Be responsible for the care coordination of clients services to ensure that they are person centred and  in line with their care plan.
  • Undertake client reviews in accordance with wellness and reablement principles, current policies and procedures and relevant legislation and guidelines. 
  • Lead and manage a team of direct care staff to deliver person-centred, in-home services including home care, personal care and respite care, to older people, people with a disability and their carers.
  • Provide strong leadership and support to staff to thrive during change, ensuring staff resources are maximised, and work priorities are aligned to organisational and departmental directions and objectives.
  • Reallocate resources, assist in program development, analyse service data and identify  continuous improvement to optimise financial resources, service outcomes and consistent work practices.
  • Contribute to the strategic service delivery planning and vision which delivers a flexible and innovative model of service for clients and the wider community now and into the future.
  • Provide supervision and support for Direct Care Staff.
  • Work collaboratively with staff to create a positive, future focussed culture.Manage up and advocate on behalf of the Direct Care Staff regarding issues, concerns, and needs, and work with relevant staff to have these matters dealt with and completed in a timely manner.

About you

  • Degree or Diploma or substantial relevant experience in aged care, health, social welfare, community development or other related fields
  • Demonstrated experience in holistic assessment process and person-centred care coordination and client reviews in line with the Active Service Model, reablement and wellbeing approach.
  • Demonstrated understanding of the needs of an ageing population, vulnerable community members, people with disabilities and their carers
  • Knowledge of relevant Commonwealth and State Government policy, frameworks, funding agreements and service standards; and person-centred service provision in line with reablement and wellbeing approaches.
  • Demonstrated ability to lead, develop, train and motivate staff with an emphasis on performance, continuous improvement, innovation and collaboration within a changing environment
  • Proven experience in maintaining and developing relationships across multidisciplinary teams within the business using high level written and interpersonal consultation, negotiation, and communication skills
  • Willingness to undergo pre-employment background checks including: National Criminal History Check, Working with Children Check, Eligibility to Work check
  • Covid-19 Vaccination in line with State Government directives for those on the Authorised Worker/Authorised Provider list
