The City of Port Phillip is seeking a suitably qualified and experienced individual with knowledge of the Aged and Disability Reforms to lead the Community Support unit which includes the following services:

  • Home support
  • Children's respite
  • Food services
  • Property Maintenance


The successful applicant will be committed and capable of leading change processes while continuing to ensure high quality service delivery and value to clients. They will have a breath of experience in managing the delivery of Health and/or Community Services and be a highly effective and capable professional. City of Port Phillip is looking for an effective leader with a commitment to achieving high quality outcomes through a passion for developing high performing teams. As Coordinator Community Support your key responsibilities will be to:

  • Manage, lead and resource a multi-disciplinary team to deliver high quality Home care services as part of the Commonwealth Home Support Program (CHSP) and Home and Community Care for Younger People (HACC FYP) and ensure Council priorities are actioned.
  • Provide advice and lead change management processes in relation to the Aged Care Reforms and the National Disability Insurance Scheme, particularly regarding changes to CHSP and HACC services
  • Promote and facilitate a range of continuous improvement initiatives
  • Manage and oversee budget and project deliverables including the development of key policies or strategies
  • Work as a key member of the Access and Ageing leadership team to guide the department to achieve Council's goals.
