About the position

The Community Care Manager is responsible for:

  • Managing the day to day operation of Community Care including Meals on Wheels, Community Transport, Respite and Social Support Services; and action referrals to My Aged Care and from My Aged Care and brokerage from other services
  • Managing the provision of Community Care Services to residents of Gilgandra Shire which meet the Commonwealth Home Support Programme and Good Practice Guide and National Disability Standards
  • Initiating planning and implementation for a coordinated community care service to residents of Gilgandra Shire through a single access point within the Community Care Centre.
  • Taking a pro-active role in local and regional planning as a member of the Gilgandra Shire Council Aged Care Committee, Multi-Disciplinary Aged Care Meeting, Orana Community Services Forum and Western Region Food Services Forum, Community Transport Forum, Meals on Wheels Association, Community Transport Organisation and Interagency Committee.

Essential

  • Experience in delivery of community services or similar
  • Demonstrated ability to manage and supervise staff and budgets
  • Knowledge & understanding of Commonwealth Home Support program and disability standards.
  • Demonstrated ability to work as part of a team.
  • Proven record of quality care delivery
  • Commitment to WHS & EEO principles.
  • Excellent communication skills both written and verbal.
  • Proficiency in use of Microsoft suite of applications
  • Proven support plan experience
  • Current Class C Drivers Licence

Desirable

  • Understanding of Aged Care legislation and regulations
  • Recognised qualifications in Management or equivalent experience
  • Financial acumen especially relating to acquittal of grants and other funding
  • Experience working with and empathy towards the elderly, disadvantaged and indigenous.
  • Current First Aid Certificate.


Salary: $38 per hour
