Who is the City of Port Phillip? About your new role… As a member of The City of Port Philip’s Independent Living Team you WILL make a difference. You will lead a team of service navigators and service support officers to help people access the aged and community care services that enable them to live independently at home and in the community. You will provide advice, assess needs and coordinate service provision for individuals and their carers within the City of Port Phillip. Your contribution will ensure older adults and people who need assistance to live independently are able to maintain their independence at home and in the community. Your enthusiasm, passion and commitment to providing high quality client-focussed in-home support will help people feel respected and encouraged to make choices about their services.
