Who is the City of Port Phillip?

The City of Port Phillip is an inner-city council with around 1000 employees who deliver more than 100 programs and services to our residents, businesses and visitors. About your new role… 

As a member of The City of Port Philip’s Independent Living Team you WILL make a difference. 

You will lead a team of service navigators and service support officers to help people access the aged and community care services that enable them to live independently at home and in the community. 

You will provide advice, assess needs and coordinate service provision for individuals and their carers within the City of Port Phillip. 

Your contribution will ensure older adults and people who need assistance to live independently are able to maintain their independence at home and in the community.

Your enthusiasm, passion and commitment to providing high quality client-focussed in-home support will help people feel respected and encouraged to make choices about their services.

What does success look like in this role?

  • You will be an active leader of people
  • You will be excited to collaborate across the Independent Living Team, Council and the broader service network
  • Your well-supported team will be empowered to provide information and advice to help people navigate the service system
  • Clients receiving services will be engaged and confident in the care they receive from your team

The things we really need from the person in this role: 

  • Tertiary qualifications in the behavioural sciences or a health-related discipline (mandatory)
  • Demonstrated leadership experience
  • Demonstrated experience in assessment of client needs
  • Kindness, care and great communication skills
  • Sound, practical understanding of client-focussed service delivery

Some Mandatory Requirements that you will need for this role are:

  • Current Working with Children's Check Card
  • Police check
  • Victorian Drivers Licence and VicRoads licence verification
  • Current first aid and CPR certificate

  • evidence of mandatory qualifications/registrations/licences, 
  • proof of their right to work in Australia,

  • agreement to complete a National Police Check via City of Port Phillip’s Provider
  • evidence of a Working with Children Check (employee type with City of Port Phillip registered as the organisation),
  • Evidence of COVID-19 vaccination in line with CHO Directions.
