About Council’s Aged & Disability Department Moonee Valley City Council's Aged & Disability Services Department provides a comprehensive range of support services, community linkage, advice and referral to assist residents who are either frail aged, have a disability or who are a primary carer, to remain connected and safe in their home and chosen communities. The services are vital to the quality of life enjoyed by individuals and families within Moonee Valley and operate as part of a wide-ranging service system involving the State and Federal Government, non-government and community-based organisations. The Home Care Package Program Case Manager practices within this framework. The Opportunity This position will see you planning, organising and coordinating the delivery of consumer-directed care through timely case management for eligible residents within the scope of Moonee Valley’s Healthy Ageing strategy. Reporting directly to the Senior Coordinator, you will support Moonee Valley as an ‘age-friendly city’ through initiatives that promote community inclusion and acceptance, healthy and active living, restorative care and a broad range of appropriate support services.
