Social Support Coordinator

Council is currently seeking a motivated Social Support Coordinator to lead the planning and delivery of programs that maintain and enhance the physical, emotional and social well-being of older persons, frail aged and people with disabilities based on their individual needs and interests.

• Relevant qualifications and/ or experience in the Community Services sector.
• Experience in working with older adults and/ or people with a disability.
• Ability to develop and maintain effective relationships with a range of stakeholders.
• Have flexibility and innovation within the work environment.
• Demonstrated ability to manage staff and lead teams under a goal centered approach, thus enabling participants to achieve their identified goals.
