THE POSITION
Working in Council’s Life Choices Support Services, this position performs a key role by working with consumers, their carers and person responsible to facilitate the planning and implementation of person centred quality services which enable the consumers to maintain their dignity, respect and ability to live independently doing the things they choose.
Reporting to the Coordinator Activity and Lifestyle Support, you will:
- Provide Coordination of Supports for NDIS participants to set up services consistent with their goals, NDIS plan budget and Councils NDIS Registration.
- Demonstrate and apply a strong understanding of person-centered service coordination principles and the ability to develop care plans incorporating consumer goal setting including enablement, and realistic short- and longer-term goals and exit strategies.
- Assist with planning individual service delivery to ensure the provision of an informed, coordinated and quality service as agreed by the consumer and / or their person responsible. Prepares Service Agreements for services provided with NDIS, CHSP and HCP funding.
- Consult with consumers and their person responsible to develop and implement recreational, educational, social, cultural, health or other plans, targeting the consumer's specific wishes and giving consideration to the safety and risk of all concerned.
- Promote the availability of aged and disability services throughout the service area by establishing networks and identifying and utilising appropriate avenues to raise public awareness, in consultation with the Coordinator Activity and Lifestyle Support.
- Maintain Home Care Package budgets of up to approximately $55,000. Managing all aspects of supports and services for consumers with Home Care Packages from Level 1 to Level 4 in accordance with current Government guidelines.
- Negotiate services and equipment provision for consumers, offering choice but meeting individual budgets for specified funding.
- Facilitate and coordinate consumers' access to a range of services, with due regard to confidentiality, from a variety of medical and allied health providers so as they each receive the optimum person centred care available.

WHO WE ARE LOOKING FOR
Specifically, you will have:
Essential
- Certificate IV in Community Services (Community Work, Aged Care or Disability Work) or other relevant discipline, or an equivalent combination of qualifications, training and/or experience.
- Sound knowledge and understanding of current practices, standards and guidelines in the community care sector and specifically those relevant to service provision for people who have a disability and people who are older, plus demonstrated practical experience.
- Experience working with people who have dementia and people who exhibit behaviour that is challenging.
- Cultural awareness and sensitivity.
- Effective oral and written communication skills, including community consultation.
- Sound computer skills, in particular data entry and word processing, MS Excel and FileMaker Pro client database software.
- Class C driver's licence.
Desirable
- Demonstrated skills and knowledge related to budgeting and accounting with associated experience.
- Qualifications and / or relevant experience related to social work and / or working with people who suffer cognitive impairment and / or who are considered vulnerable.
- Experience in formulating person centred Care Management plans.
- Current First Aid Certificate.
- Current NSW Working With Children Check.
