THE POSITION

Working in Council’s Life Choices Support Services, this position performs a key role by assisting the Coordinator Direct Support to direct and coordinate the direct support team in the delivery of person-centred and quality services of the consumers’ choice, to enable consumers to maintain and enhance their independence and control.

Reporting to the Coordinator Direct Support, you will:

  • Demonstrate and apply an understanding of rostering and the ability to facilitate person-centred direct support services in line with each consumer’s/participants choice and identified need.
  • Demonstrate the ability to guide staff to focus on maintaining, enhancing or restoring consumers’ independence wherever possible.
  • Demonstrate and apply an understanding of compliance and continuous improvement and assists the Coordinator Direct Support in the ongoing evaluation and continuous development and improvement of the service.
  • Establish and maintain consumer and service records as directed by the Coordinator Direct Support.
  • Support and supervise direct support staff and maintains relevant files and rosters to ensure that services and activities are adequately staffed to meet consumer and participant needs as directed by the Team Leader Direct Support.
  • Ensure that consumers are informed about Life Choices – Support Services by making relevant and current information available to them.
  • Monitor and review service provision and care plans as instructed by the Coordinator Direct Support.

WHO WE ARE LOOKING FOR

Specifically, you will have:

Essential

  • Sound knowledge and understanding of current practices, standards and guidelines in the community care sector and specifically those relevant to service provision for people with a disability and people who are older, plus demonstrated practical experience
  • Well-developed organisational skills and the ability to work independently to plan and coordinate activities and tasks involving multiple resources
  • Sound knowledge of Work Health Safety and risk in relation to the delivery of direct support services
  • Effective oral and written communication skills as required to achieve outcomes
  • Sound computer skills, in particular data entry and word processing, MS Excel and FileMaker Pro client database software
  • Current Class C driver’s licence

Desirable

  • Certificate IV in Community Services (Community Work, Aged Care or Disability Work) or other relevant discipline
  • Experience working with people with dementia and people who have challenging behaviours
  • Experience working in Rostrit or other staff rostering program
  • Current First Aid certificate
  • Current NSW Working With Children Check
