About the Role  

  • Deliver flexible, reliable and efficient home-based services to clients.
  • Provide supervision, support and guidance to Community Care Support Workers.
  • Maintain community care administration including electronic rostering, client information and office systems (HACCPAC).
  • Lead client care plans and direct service provision within the team.
  • Liaise with clients, contract service providers and Community Care Support Workers to ensure an efficient and effective service delivery.

About you 

  • Become an integral leader with the ability to engage and motivate a team of Community Care Support Workers to deliver contemporary best practice programs and services.
  • Proven ability to build relationships with a variety of internal / external stakeholders to establish and maintain positive interactions.
  • Act with empathy and in a responsive, sensitive and respectful nature toward people.
  • Knowledge and understanding of standards and guidelines including Commonwealth Home Support Program (CHSP) and Victorian Home and Community Care Program for Younger Persons (VicHACCPYP).
  • Dedicated to providing a customer/client centric approach.
  • Minimum Cert IV Community Services Qualification (desirable)
