This position presents an exciting opportunity to work within the Kids Guided Personalised Service (Kids GPS) Care Coordination team at The Sydney Children's Hospitals Network and in partnership with Western Sydney Primary Health Network (WentWest). This role will work as an Aboriginal Health Worker on the 'Linking Kids Care' Project, which is a joint initiative between The Sydney Children's Hospitals Network (SCHN) and Western Sydney Primary Health Network (WSPHN) - WentWest. 

The Aboriginal Health Worker will aim to support better access, liaison, health promotion and preventative health services to Aboriginal communities. A key focus of the position will increase engagement of Aboriginal children, young people and families with health services in their local community and facilitate the link between hospital, Primary Health, community health services and relevant non-health agencies for a specific cohort of patients enrolled in the program. 

This position is Temporary Full Time up to 30/06/2019



What you'll be doing
The role will focus on a vision for Aboriginal children and families having access  to high quality, evidence based and culturally-safe child and family health services to support optimal health, development and wellbeing. 

This role will work as an Aboriginal Health Worker on the 'Linking Kids Care' Project, which is a joint initiative between The Sydney Children's Hospitals Network (SCHN) and Western Sydney Primary Health Network (WSPHN) - WentWest.  

The Aboriginal Health Worker will aim to support better access, liaison, health promotion and preventative health services to Aboriginal communities. A key focus of the position will increase engagement of Aboriginal children, young people and families with health services in their local community and facilitate the link between hospital, Primary Health, community health services and relevant non-health agencies for a specific cohort of patients enrolled in the program.  

The Aboriginal Health Worker will support the provision of integrated care between the Hospital, primary health and community health services for Aboriginal children, young people and families.  This role will work as part of the Kids Guided Personalised Services (Kids GPS) Care Coordination team in close collaboration with the care coordinators in the role at The Children’s Hospital at Westmead (CHW) and Sydney Children's Hospital Randwick (SCH) promoting the importance of Aboriginal children and young people requiring access to high quality health care that is delivered by a workforce and service system that is culturally responsive and inclusive of the unique and individual needs of Aboriginal communities. The role will be based at CHW but will be required to participate in home and community visits and attend the WentWest Offices (WSPHN) as required. 

The position will actively participate in community forums and events including Aboriginal network meetings, related to the health, social  and wellbeing of children, young people and their families, and continue to advocate for equitable health outcomes.  

The Aboriginal Health Worker will work in collaboration with: Aboriginal children, young people and families; Community Health; Western Sydney Local Health District (WSLHD); General Practitioners; WentWest Division of General Practice; Aboriginal health and support services within the Hospital and the community, particularly GWAHS; Private Health Care Providers; Non-Government Organisations (NGOs); HealthOne General Practice Liaison Nurses (GPLNs); and SCHN multidisciplinary clinicians. 
  • This is an Aboriginal identified position and the application must be of Aboriginal and/or Torres Strait Islander descent (pursuant to Section 14 (d) of the Anti-discrimination Act 1977).
  • Holds or aspires to hold a minimum Certificate III qualification in Aboriginal Primary Health Care or a minimum Certificate III health qualification in the area of care in which the Aboriginal Health Worker works
  • Demonstrates understanding of the historical and contemporary socio-political experiences of Aboriginal people in Australia.
  • Demonstrates good communication skills with children, young people and their families to support engagement in care coordination.
  • Demonstrated knowledge of social determinants and key initiatives aimed at achieving equitable health outcomes for Aboriginal children and young people.
  • Proven ability to engage, participate in and effectively communicate with multi-disciplinary and broader health care teams.
  • Demonstrates the ability to navigate computer operating systems including those required to access and record patient information and data.
  • Holds a New South Wales drivers Licence
