About this role

  • Ongoing part time position at 0.7 EFT

This role provides social work leadership and expertise to the state wide Victorian Paediatric Palliative Care Program (VPPCP) and is based within Monash Children’s Hospital Allied Health. The VPPCP is an alliance between The Royal Children’s Hospital, Monash Health and Very Special Kids Inc.

The VPPCP team is comprised of medical, nursing, occupational therapy, social work and admin staff. The Program uses a multi-disciplinary consultation- liaison model of care that provides consultation to health professionals across Victoria in the acute paediatric and community palliative care sectors. Primary responsibility for day-to-day patient care remains with the treating unit and community palliative care team. The VPPCP also manages a Flexible Funding Program which provides funding for services and equipment which families cannot access through other programs.

In addition to clinical consultation and liaison, one of the aims of the VPPCP is to build the capacity of the broader health care system to provide the best possible palliative care to children and their families regardless of diagnosis. This is done through education, research and advocacy.

This is a leadership role. Key functions include:

  • Provision of an integrated approach to children’s palliative care in Victoria
  • Participation in monthly VPPCP team meetings
  • Capacity to visit Monash Children’s patients at home or in the children’s hospice (Very Special Kids)
  • Education of health professionals

The MCH VPPCP social worker is expected to provide secondary consultation to medical, nursing and allied health staff who are caring for children with palliative care needs across the state of Victoria. They are expected to lead educational initiatives in the field and to contribute significantly to service development.

Social Work within Monash Children’s Hospital Allied Health provides specialised services to a broad range of patients and families in both inpatient, day admission and outpatient settings.

As a Social Worker based at Monash Health you will be required to participate in the On-Call Service for sudden and unexpected deaths, children at risk and perinatal loss. This role will also provide clinical supervision to specified members of the Monash Children’s Hospital Social Work Team.

Monash Children’s Hospital is one of Australia’s leading providers of integrated children’s health services, with more than 30 specialist services and programs. We have Victoria’s largest Neonatal Intensive Care Unit and provide leading paediatric services in many areas including Rehabilitation & Oncology

As a senior social worker you will have a good knowledge of resources and have a great understanding of the assessment and intervention frameworks used to guide practice including the Children’s Youth and Families Act and appropriate referral networks. You will be able to demonstrate skills in psycho-social assessments, problem solving, communication and discharge planning. You will develop and maintain strong links with universities and your Monash Health social work colleagues.

What we’re looking for in you

  • 7 years’ experience including clinical expertise and clinical leadership skills recognised in closely related field of practice
  • Approved degree and evidence of eligibility for membership of AASW is essential
  • Post graduate qualification in Social Work or a relevant field preferred
  • Demonstrated CPD accreditation to AASW requirements
  • Current Working With Children Check
  • A good working knowledge of hospital and community based services
  • Demonstrated knowledge, skills and ability to respond to  grief, loss and bereavement
  • Demonstrated experience in complex case management

Location

The role will be based at Monash Children’s Hospital. Monash Children’s Hospital is a purpose built facility in the south eastern suburbs of Melbourne. Travel will regularly be required to other community locations.
